Long term health condition management during Covid-19

The HSE has identified that adults living with long term health conditions (LTHCs) - such as COPD, asthma, diabetes and cardiovascular conditions - are at increased risk of serious illness due to COVID 19. 

Evidence indicates that for people with pre-existing LTHCs, enhanced management of these conditions can directly contribute to better outcomes from COVID-19 infections.  Evidence also indicates that LTHC management is at risk of neglect when normal healthcare services are disrupted such as during this pandemic.  https://www.cebm.net/covid-19/supporting-people-with-long-term-conditions-ltcs-during-national-emergencies/

The National HSE Self-management Support Programme has developed a booklet to support adults in the management of their diabetes, COPD, asthma and cardiovascular conditions during the COVID-19 pandemic.  This booklet is called Minding Your Long-term Health Condition During Coronavirus (COVID-19). It provides practical and clear information to support people to manage their long-term health condition and to stay well. 

This resource has been approved by the National Clinical Care Programmes for Diabetes, Respiratory and Heart Failure, and by the Health Protection and Surveillance Centre.
